To join, fill out the Lebanon FireMed® form below. Send the completed form, along with your payment, to the Lebanon Fire District by mailing it to 1050 W. Oak Street, Lebanon, OR 97355. If you're already a member, don't worry – you will get renewal reminders before your membership expires.
